What to Expect (Job Responsibilities):
- Coordinate ongoing care for TCHATT patients in collaboration with program clinicians
- Serve as a liaison between patients and clinicians
- Disposition TCHATT cases upon discharge from services
- Develop and coordinate care plans for patients
- Assist with the development and maintenance of the mental health resource database

What is Required (Qualifications):
- Bachelor's degree in Psychology, Social Work, or Mental Health/Behavioral Science
- Criminal Justice degree considered with 15 hours of coursework in psychology, social work, and/or counseling
- 2 years of MH case management experience
- Competency and skill in communicating with patients, families, and school personnel
- Participation in team meetings and providing status updates
- Proficient computer skills

How to Stand Out (Preferred Qualifications):
- 1 year of experience working with families
