Coordinate, facilitate and consult with community-based, culturally-specific, client-centered coordinated care with the following systems and agencies: schools, child welfare, judicial, health, correctional, intellectual developmental disabilities, and mental health service providers.
Coordination focuses on addressing barriers to access, collaboration, monitoring client progress in services, and developing Intensive Care Coordination to align systems, identify strengths and needs, refer for medically appropriate mental health services and coordinate other system services including Wraparound to provide a comprehensive plan to meet the complex needs of a child and family.
Maintain timely and accurate documentation about each member.
Maintain tracking and data entry for program reporting and evaluation.
Utilize Hospital System Electronic Medical Record, CareOregon Electronic Care Management System and other systems as needed to gather information needed to provide comprehensive care coordination for members.
Assess and identify care coordination needs through member contact, review of available clinical and utilization information, contact with providers, community partners and other member supports as appropriate.
Identify risk factors with service & support needs and develop an individualized care plan for the member based on the needs assessment.
Ensure that racial/cultural/ethnic factors, including any factors related to non-dominant culture affecting behavioral health and treatment are considered in the assessment and screening process.
Graduate degree in psychology or
Graduate degree in social work or counseling or
Graduate degree in a behavioral science field
Must have a valid Driver’s License.
This position requires a Background Investigation, which may include being fingerprinted.
Advanced knowledge of issues young children and families witnessing or being impacted by violence, aggressive behavior, depression, anger problems, behavior problems, attention problems, dealing with grief, etc
Respect and appreciation for cultural and ethnic diversity.
Ability to apply an equity, intersectional and anti-racist lens.
Ability to provide information/referrals to community mental health providers.
Crisis intervention, including risk assessment of clients who may be involved in potentially life threatening situations.
Advanced skills in managing sensitive information and ability to support the community.
Knowledge of cultures, customs and social experiences of communities experiencing marginalization and barriers to opportunity.
Understands , racial, cultural and ethnic factors, including systemic advantage or disadvantage; acculturation, intergenerational immigrant family challenges, effects of trauma during immigration, war, separation from family and/or primary supports, detention, refugee camps, etc.; generational trauma, and any factors related to non-dominant culture affecting mental health and treatment.
Ability to exercise good judgment, diplomacy, and tact in dealing with sensitive, complex, confidential information and situations, in addition to acting professionally, and empathically, with individuals, internal/external partners and communities.

We’re seeking social workers and mental health professionals who represent the communities we serve and are dedicated to removing barriers to youth and families who experience intersectionality to join our team as the Mental Health Consultant - Youth Intensive Care Coordinator . As the Intensive Care Coordinator (ICC) you are responsible for developing and implementing member-centric, individualized care plans and providing telephonic and community-based care coordination for youth with high health care needs, including youth with complex behavioral concerns, severe and persistent mental illness, substance use disorders, and/or receiving facility based, in-home or community-based psychiatric services. You will utilize your clinical expertise of behavioral health conditions and in depth knowledge of the system of care and community resources to provide care coordination that is client centered, trauma informed, culturally responsive, and linguistically appropriate. In addition, you will provide intake, consultation, assessment, and intervention on behalf of children, adolescents, transitional aged youth, and families who exhibit severe emotional disturbances, substance abuse and/or criminal behavior. You will facilitate community-based, culturally-specific, client-centered coordinated care in collaboration with systems of care partners such as; education, child welfare, corrections, health care, family, and community (this includes consultation and training).
