- join the friendly Adult Community Mental Health services team
- act as care coordinator for those service users settled in placements in south west Hertfordshire who require an annual review of their care
- acting as a point of contact for care providers
- act as a point of contact to the HPFT placement service
- supervise a Band 4 STaR worker dedicated to providing support to those in Community placements
- have a good understanding of social care and the Care Act
- have the ability to assess need and commission packages of care to promote strength based interventions with a focus on the Recovery model
- work with a Support Time and Recovery worker as part of the Adult Community Mental health staff team
- liaise closely with key staff members in the Continuing Care and Placement Service
- coordinate annual placement reviews
- develop and foster good relationships with placement providers and the Adult Community Mental Health Service
- promote and deliver the Connected Lives Programme.
- You will require a professional registration for social work, nursing or occupational therapy to undertake this role and to act within the appropriate professional code of conduct at all times.
- You will act as the Care coordinator for service users in funded social care and health placements.
- You will be responsible for ensuring that an individualised comprehensive assessment and Care Management service is effectively delivered in line with operational and service delivery requirements.
- Responsibilities include the clinical and operational supervision of Support Time and Recovery staff and students as delegated by the team manager.
- You will lead for delivering a quality service that is effective and efficient in line with the organisations objectives and performance indicators.
- You will contribute to strategic service development, working collaboratively with other agencies.
- You will work co-productively with service users in placements with complex health presentations, promoting their recovery and developing their support plans to enable them to lead fulfilling lives.
- You will work in partnership with Health and social care professionals from other agencies and organisations
- You will work at all times to promote the safety and the wellbeing of service users and their families/carers.

- RMN or Dip/Bsc (Hons) in Mental Health Nursing Or HCPC Registered Social Worker / occupational Therapist
- Training in principles and application of outcome focused assessments and personalisation
- Completion of metentorship /preceptorship Course/ENB equivalent, or intention to complete
- Up to date knowledge of relevant legislation, ethical issues, strategic frameworks and current guidelines regarding the range of presenting problems in adult mental health, their implications for both practices and clinical management
- Evidence of continuing professional development
- Clinical risk assessment and management in a community setting.
- Experience and knowledge of applying the Care Act in practice
- Excellent written and oral communication skills
- A form of own transportation
- Computer literacy
- Experience of working in a range of settings relevant to the post
- Extensive post registration experience with at Band 5 or equivalent
- Experience of working in mental health Community/day care/ Crisis assessment home treatment teams services
- Experience of working in situations of crisis and emotionally demanding environments
- Experience and knowledge of safeguarding process
- Experience of commissioning social care packages
- Experience of working with people with severe and enduring mental health problems in a community setting.
