      Job summary

      The successful candidate will work within the MDT to provide assessment and interventions for those referred to the service. A vital part of the role is the Triage function of the service, screening referrals and offering advice, guidance and signposting where necessary.

      The candidate will be based within the Central Locality of Islington and work in a recovery-orientated approach, offering joint consultations with the MDT, as well as wider networks including GPs, interface services and the voluntary sector.

      Phlebotomy qualification and/or experience is desirable.

      Main duties of the job

      - Triage; screening of referrals and other correspondence such as Safeguarding Alerts, Police Reports and enquiries from GP's, LBI Housing/Social Care and patient/carer emails

      - Assessment of current care and support needs

      - Interventions based on speciality of practice, including joint working with other C&I Services

      - Engagement in service delivery and development; attendance to MDT meetings, supervision and other training opportunities.

      About us

      The Islington Practice Based Mental Health Service has grown over the last four years from a pilot service covering five GP practices to a fully commissioned service covering all GP practices in Islington. The team has a broad remit, working as one of the key points of access to secondary care community services in the Trust, and receives referrals from GPs, adult social care, housing, the police and voluntary sector services. The service sits on the boundary between primary and secondary care and aims to better integrate physical and mental health by working more closely with our GP colleagues in a flexible, creative and collaborative way.

      The partnership betweenBarnet, Enfield and Haringey Mental Health NHS Trust (BEH)andCamden and Islington NHS Foundation Trust (C&I)is going from strength to strength since it was originally established in 2021 forming theNorth London Mental Health Partnership.

      The postholder will need to be comfortable working in an environment of complex matrix management arrangements and will at all times behave and align with our Trusts' values and cultural pillars.
