      As part of developing our liaison services within UHL we have funding for a Band 6 Mental Health Practitioner post within the older persons component of the Mental Health Liaison Services.

      The team is based in the Leicester Royal Infirmary and provide assessments for over 65s presenting in the acute medical setting with primary or co-morbid mental health problems. We offer an urgent assessment service to people attending the Emergency Departments in times of mental health crisis, and an assessment and liaison service to the medical wards across the 3 UHL sites. The service operates 9am-5pm seven days a week.

      Main duties of the job

      The successful candidate will be responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of patients according to changing healthcare needs and varying levels of complexity and for the planning, allocation and the delegation of duties and care priorities to the multi-disciplinary team.

      The post holder will provide advice, education, support and information to patients and their relatives / carers as required and involve patients, relatives / carers in the ongoing planning and delivery of care. Establishing and maintaining effective communication with patients and relatives / carers.

      To recognise, prioritise and respond appropriately to urgent and emergency situations, receive reports from team members on the progress of own patients on the designated caseload as and when necessary.
