      Description

      PRINCIPAL MISSION:

      The MCT Social Worker provides support, services, and treatments necessary to provide integrated crisis response, crisis stabilization interventions and crisis prevention activities for the SBH Health System Department of Psychiatry’s Mobile Crisis Team (MCT). This role requires clinical and administrative expertise and ensures quality care for diverse populations throughout the Bronx County.

      PRINCIPAL DUTIES AND RESPONSIBILITIES: RESPONSIBILITIES

      1. Promotes a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c).

      2. Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.

      3. M akes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner, exercising courtesy and tact. Effectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion.

      4. Demonstrates the standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.

      5. Provides support, services, and necessary treatments for the ongoing Mobile Crisis Team (MCT).

      6. Provides treatment to ensure symptom and harm reduction and safely guides people in acute crisis to appropriate crisis stabilization and detoxification support/services.

      7. Provides immediate telephonic response to assess crisis and determine the risk, mental status, medical stability, and appropriate response; provide face-to-face response as needed.

      8. Makes referrals for emergencies or continuing care or may suggest treatment alternatives to facilitate services in the community in a safe environment.

      9. Develops crisis plans before discharge for new patients and/or revise existing crisis plan components as appropriate.

      10. Monitors or coordinates movement across levels of care, directly to the person and family.

      11. Acts as a liaison to local Police Departments, Probation offices, shelters, crisis services etc., to inform them of MCT services and collaborate in the care of individuals in mental health crises.

      12. Advocates for high quality care of individuals with mental health concerns in the community and child/family serving systems.

      13. Documents all client contacts in accordance with institution and DOH guidelines as appropriate.

      14. Other duties as assigned.

      Qualifications

      QUALIFICATIONS:

      • Master’s degree in social work from an accredited college or university.
      • Must be licensed NYS LMSW.
      • Must have a minimum of one (1) year of experience in providing crisis management services in the following settings: assertive outreach, assertive community treatment, emergency department etc.
      • Must have two years of prior experience in the mental health field.
      • Bilingual English/Spanish preferred but not required.
      • Able to work flexible hours and days - including weekends/evenings/holidays.
      • Experience with diverse populations and social service systems.
      • Experience with EPIC or other EMR systems.
      • Mobile Crisis Team and/or CPEP experience strongly preferred.
      • Position requires a valid NYS driver’s license with a clean driving record.
      • Travel throughout the Bronx County is essential.
