      Job Description:

      Description: The Mobile Health Coordinator - Acclaim will support the Acclaim Street Medicine Program and Intensive Primary Care by planning and executing community outreach initiatives for both programs. This position is a frontline public health worker who will act as a patient advocate and liaison to enhance collaboration between the health care system, social services, and the community partners. The Mobile Health Coord - Acclaim also builds individual and community capacity by increasing health knowledge and self-sufficiency, which will lower health disparities in Tarrant County by providing access to services and creating an effective system of chronic disease management for underserved populations. This role requires a close understanding of the community served as well as JPS Health Network services and programs. If working with Acclaim Street Medicine there will be times when the mobile unit/van for transportation will be utilized. The Mobile Health Coordinator role is expected to drive their own vehicle for transportation.

      Typical Duties:

      1. Develops partnerships within JPS Health Network, Acclaim and community organizations.

      2. Conducts intake interviews with patients, including enrolling and/or referring patients into programs and assists patients with completing applications and registration forms. Assists with eligibility determination, enrollment, and follow-up with uninsured patients.

      3. Provides assistance with transportation resources as appropriate.

      4. Works collaboratively with other clinical personnel assigned to the same patient to coordinate care.

      5. Recruits participants and facilitates self-management, nutrition, psychosocial support groups and physical activity sessions according to State approved curriculum.

      6. Conducts needs assessments, develops patient plans, conducts and/or navigates patient to appropriate interventions designed to improve access to health care and/or health status.

      7. Provides referral and follow-up services or otherwise coordinates and proactively identifies and enrolls eligible individuals in Federal, State, and local private or nonprofit health and human services programs.

      8. Provides teaching and education on direct services such as Biometric screenings, first aid, health promotion, disease prevention, and self-management.

      9. Provides culturally appropriate health education, information, care coordination and outreach in community-based settings such as homes, schools, clinics, shelters, local businesses, and community centers.

      10. Leads interdepartmental and interagency meetings regarding high Emergency Department (ED) users and vulnerable outreach clients.

      11. Collects and monitors data on ED usage, service agency usage, etc.

      12. Identifies and resolves issues that create barriers to care for specific individuals.

      13. Attends outreach events and communicates with community partners to plan, organize, and coordinate outreach activities.

      14. Trains and coordinates staff, students, etc. and maintains a calendar of events. Prioritizes patients for outreach.

      15. Monitors health needs and reinforces treatment regimens.

      16. If working with Acclaim Street Medicine there will be times when the mobile unit/van for transportation will be utilized. The Mobile Health Coordinator role is expected to drive their own vehicle for transportation.

      17. Performs other related job duties as assigned.

      Qualifications:


        Required Education and Experience:
      • High School Diploma, GED or equivalent.
      • 3 plus years’ experience with community outreach and volunteer coordination.
      • Previous experience with Electronic Medical Records such as EPIC.
      • Understanding of the culture, language, socio-economic status, and life experiences of the community served.

      • Required Licensure/Certification/Specialized Training:
      • Successful completion of DSHS Certified Community Health Worker Certification within 6 months of hire.
      • Valid Texas Driver’s license

      • Preferred Education and Experience:
      • Bachelor's degree in healthcare, education, business, psychology, computer science or related field.
