      LOCATION: Home site will be at our West Fairfield Clinic; travel to all Fairfield and Napa sites required.

      SCHEDULE: Full Time; Monday- Friday

      PAY RANGE: $30.40 to $37.15 hourly

      JOB SUMMARY/OVERVIEW:

      The Mobile Health Program Coordinator provides leadership for the mobile health staff, maintains the mobile unit to ensure the safety of the vehicle and manages the day-to-day responsibilities of the mobile unit. The Program Coordinator will also ensure that Communicare+OLE is building partnerships and collaborates with various organizations throughout the service area. The Program Coordinator will be responsible for driving the mobile van from its storage/parking place to places of service and will prepare and set up the vehicle to be ready for services. The coordinator will work in the field to support the full team, taking on various roles and responsibilities. The Coordinator will travel extensively with the mobile health team in both Napa, Solano and Yolo counties to provide care to the community at large, including but not limited to, farmworkers, unhoused, students, seniors, and other vulnerable populations.

      MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

      1. Education:
      • Bachelor's degree in public health, healthcare administration, or related field preferred
      • Must have a Medical Assistant Certification and a minimum of 1 years’ experience as a medical assistant.
      1. Experience/Lived Experience:
      • Minimum of 2 years of experience managing a health clinic or program strongly preferred.
      • General knowledge of community health center’s philosophy and operating principles
      • Ability to communicate effectively with various stakeholders including patients, community partners, the community, and staff and management
      • Know and understand local health community needs, resources, and organizations to help support and improve health outcomes
      1. Special Skills/Training:
      • Able to communicate fluently in English and Spanish (Preferred)
      • Strong computer and data entry skills preferred, as well as knowledge of Microsoft suite (Word, Excel, Power Point, Outlook, etc.), and healthcare data management systems
      • Valid CA Driver’s License required

      ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

      1.

      Ensure timely and efficient delivery of mobile services to target populations; oversee the daily schedule/calendar and operations of the mobile program; ensure that Mobile Health Unit is out in the community at least 4-5 times per week; Coordinate and obtain appropriate consents for treatment and documentation required from parent/guardian(s); Coordinate/oversee the scheduling of appointments for treatment; ensuring adequate number of patients are scheduled; troubleshoot and resolve inquiries and/or concerns; facilitate the transport of patients/potential patients to and from the mobile clinic. Responsible for supporting with weekly and monthly staffing coverage to ensure care is not impacted.

      2.

      Serve as a liaison between Communicare+OLE and the sites to be serviced; in coordination with the Associate Medical Director and the Mobile Health and Migrant Services Manager, identify community resources in the service area, and establish partnerships with identified organizations.

      3.

      Knowledge of community health/public health philosophy and operating principles

      Knowledge of the different community stakeholders (community partners, vulnerable and hard to reach communities, etc.)

      5.

      Ensure parking arrangements; ensure the safety of the mobile clinic and compliance with all risk management policies and guidelines; provide a secure and clean environment on the mobile unit; ensure proper handling and disposal of hazardous material. In partnership with the Mobile Health and Migrant Services Manager managing the details around Specialty RV management.

      6.

      Highly organized with the ability to manage multiple projects.

      7.

      Demonstrated ability to provide excellent customer service.

      8.

      Knowledge of Federally Qualified Health Centers

      9.

      Provide technical expertise to manage patient records through the electronic health records (EHR) system used by the organization; conduct regularly scheduled team meetings; monitor entry of data and ensure timely completion of encounters, charges and/or payments; conduct inventory and ensure required supplies and needed resources are ordered and stocked for team utilization; conduct audits of patient charts to ensure timely, consistent, accurate and complete documentation in compliance with the treatment/services rendered; track patient care outcomes and ensure completion of treatment plans; compile data and produce quantitative and qualitative program/department reports as needed; ensure patient confidentiality and EMR security at all times; maintain patient information within HIPAA standards; keep other care team members informed when situations occur that disrupts timely patient flow through site. Partner with Security officers to ensure safe services in the field.

      10.

      Drive the mobile health vehicle from the place of storage/parking to service areas, complying with all motor vehicle codes and regulations; prepare and set up the vehicle for services as well as break down at the end of the session; note any maintenance issues for the vehicle and report them to the Mobile Health and Migrant center manager; maintains a daily log of vehicle mileage and compressor hours; completes driver vehicle evaluation form weekly; maintains log of routine inspections of vehicle, daily, weekly and monthly; helps maintain the cleanliness of the vehicle as directed, including interior and exterior; turns on and off all necessary equipment at the beginning and end of each day; works to secure parking permits for the mobile vehicle as appropriate; comply with Communicare+OLE’s motor vehicle safety policies and regulations; have a valid driver’s license.

      11.

      Coordinate care team members and provider's work schedules based on hours of operation to ensure adequate coverage; arrange temporary coverage with other offices in a timely fashion to ensure continuity of the mobile health operations.

      15.

      Secures permits and necessary approvals for the mobile unit operations at different locations. Coordination of transportation, logistics, and maintenance of the mobile unit.

      16.

      Maintains accurate patient records and clinical data for the program. Generate reports on patient demographics, service utilization and KPI.

      17.

      Monitor the quality of care provided on the mobile unit, identifying areas for improvement. Identify and implement quality assurance measures to enhance patient experience and clinical outcomes.

      18.

      Serve as onsite support as a medical assistant as well as a patient access representative.

      19.

      Provide operational support to prepare for audits.

      20.

      Performs other duties as assigned.
