      SUMMARY

      Responsible for engaging individuals seeking services, conducting screening for services, documenting request for services in the agency’s electronic health record system, and linking to appropriate services. Support disposition meetings for the timely assignment of clients to the appropriate program and practitioner. Ensures compliance with legal, ethical, contractual and agency requirements and standards.

      ESSENTIAL DUTIES

      1. Engages individuals seeking services by conducting walk-in and telephone screening/interviews, in English and Spanish, and obtaining necessary information and documentation to determine program relevance and eligibility, consistent with agency protocols and county/state requirements.
      2. Determines appropriateness for agency services, and when eligible. Makes arrangements for assignment to a service practitioner.
      3. Participates in weekly disposition meetings with Behavioral Health & Wellness Department managers for assignment of clients.
      4. Provides community referrals when individuals are not eligible or appropriate for agency programs, coordinates services with other providers, and maintains resource directory of community programs.
      5. Documents request for service, screening information, disposition, and intra- or inter-agency referrals in the agency’s electronic health record system.
      6. Maintains tracking system for all contacts and requests for services for internal and external reporting purposes.
      7. Researches and corroborates information from weekly program reports Coordinates activities, clients’ status and claim status with the front end HIM billing unit, and inputs to support claiming activities
      8. Performs a variety of general clerical duties including filing, copying, collating, data entry.
      9. Establishes and maintains professional, collaborative and effective working relationships with children and families, agency staff and external stakeholders.
      10. Works collaboratively with Health Information Management (HIM) Billing /Front-End Billing, Quality Assurance Coordinators, and HIM Specialists.
      11. Ensures confidentiality and maintenance of client records in compliance with HIPAA, CII Policies and Procedures and licensure regulations.

      SUPERVISORY RESPONSIBILITIES

      • This job has no supervisory responsibilities.

      EDUCATION & EXPERIENCE

      • Six months of health information management or related experience.
      • Bachelor’s degree in child development, early childhood education, psychology, social work, or related field required.

      OTHER QUALIFICATIONS

      • Possesses a valid driver's license and state-required auto insurance.
      • This position may require some travel.
      • Ability to be flexible and work in a changing environment.
      • Spanish / English bilingual required.
      • Ability to handle confidential information.
      • Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
      • Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.
      • Sensitivity to service population’s cultural and socioeconomic characteristics.
      • Excellent verbal and written communication skills.
      • Punctual and dependable attendance.
      • Free from alcohol and drug abuse.
      • Understands CII’s basic philosophy and participates fully in carrying out its mission.

      • None required.

      • Must be fully vaccinated to comply with the California Department of Public Health (CPHD) order issued on August 5, 2021.

      COMPENSATION:

      • Hourly hire rate: $23.88
      • In accordance with CA law, the expected pay range for this CA position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
