Job summary
A fantastic opportunity has arisen for enthusiastic and dynamic clinical assistants to join The Neighbourhood Teams covering the west of Leeds.
Areas we cover are; Woodsley, Holt Park, Yeadon and Armley and the surrounds. We provide care in patients homes, whilst enabling early discharge from hospital and reducing hospital admissions.

The service runs 7am to 10pm every day with various shift patterns. We understand that people have busy lives, and we offer flexible work opportunities.
Main duties of the job
You will work in a multidisciplinary team of nurses, therapists, and self management facilitators, providing care to patients in their own home.
You will need to assess and respond to the changing needs of the patient, working within your own limits and seeking support from the wider clinical team where appropriate.
You will provide patients with clinical support for example administering medications, undertaking wound care and carrying out observations.
Please note this role does require a full Driving licence and use of your own vehicle which is essential to this job role.
