      Job summary

We are looking for a forward-thinking and motivated nurse to join the Neighbourhood Nursing teams across Lambeth and Southwark areas.

We are the first Trust in the UK to trial the Buurtzorg District Nursing Model: an entirely Nurse-led approach to patient care in the community with self-managed nursing teams. You will find yourself working in a completely different way.

You must demonstrate excellent communication and team working skills. Through clinical case management you will care for patients with long-term conditions, those at risk, end of life care needs, and those with supported self-care programmes.

There will be training and opportunities in addition to clinical supervisions and appraisals. The post holder must be able to undertake a prescribing course and advanced assessment courses once in post.

Working on a 7-day shift service (hours between 8.00 - 20.00), this post also includes rotational weekend work (maximum two per month).

We are currently trialling a new way on interviewing called transparent interviews, whereby we will provide the 60% of the non-clinical questions to the candidates 24 hours in advance - for further details on this please contact Emily.Jones4@gstt.nhs.uk - Matron for Recruitment & Retention.

Closing Date: 1st June 2025 at Midnight

  • About the company

National Health Service (NHS) is the umbrella term for the publicly-funded healthcare systems of the United Kingdom (UK). The founding principles were that services should be comprehensive, universal and free at the point of delivery—a health service based on clinical need, not ability to pay. Each service provides a comprehensive range of health services, free at the point of use for people ordinarily resident in the United Kingdom apart from dental treatment and optical care.
