Deliver comprehensive assessments and clinical management to patients across a range of clinical areas.

Work collaboratively with GPs, allied health professionals, and other clinical staff to ensure effective care pathways.

Lead in the development and implementation of evidence-based protocols and policies.

Provide leadership and mentorship to junior nursing staff and other members of the clinical team.

A registered Nurse with advanced practice qualification.

An experienced Advanced Nurse Practitioner with a focus on patient care and clinical leadership, with strong clinical expertise and the ability to work autonomously with excellent communication, problem-solving, and team-working skills.
It is a requirement for this role that you will have completed a Centre for Advancing Practice-accredited MSc Advanced Practice programme, or Completed the Centres ePortfolio route, or are currently completing one of these approved routes to obtain or have your digital badge.

Job summary
An exciting opportunity has arisen for an Advanced Nurse Practitioner to join our friendly and innovative Primary Care Network for up to 37 hours a week under the Additional Role Reimbursement Scheme, supporting the teams of our four member practices with our acute patients and joining our two current ANPs as well as a host of network colleagues including Pharmacists, Social Prescribers, Care Coordinators, working within our practices as part of our Primary Care team.
Main duties of the job
As a Network Advanced Nurse Practitioneryou will be providing expert clinical care across various settings, although we would aim to site you at not more than two of our sites so that you can become fully integrated into our teams.

Poole North Primary Care Network are four practices working together to ensure excellent health outcomes for our patients. We are friendly and supportive with PCN and practice teams working closely together in a collaborative way. Together we provide services for our 52,000 patients across 7 sites in Canford Heath, Broadstone, Merley, Corfe Mullen, Creekmoor and Oakdale.
(The location for this job has been set to Canford Heath for management purposes only but it is envisaged up to two bases will be provided in our other practice locations).
