      Job summary

      To work alongside the management team in assisting with the running of the Bromley GP Alliance services

      To be the first point of contact for the network and act as an enabler for improved health in order to improve services to patients.

      Working alongside the Crays Clinical Directors and BGPA management team in assisting with the running of the Crays PCN.

      The post holder will ensure that the network and its member practices have a strong voice and are enabled to apply authority as an essential part of the transforming healthcare environment.

      Support the Clinical Director and practice managers in the member practices.

      Main duties of the job

      Work with the PCN Clinical Directors and boards for strategic and operational planning processes to support the PCNs objectives

      Liaise closely with the practices to help understand the impact of PCN activities on practices to assist the Board to meets its objectives and to ensure coherence across the PCN.

      Provide support to the Clinical Director and deputise in their absence.

      Job Summary

      Duties and Responsibilities as a Network Manager:

      Liaise closely with the practices to help understand the impact of PCN activities on practices to assist the Board to meets its objectives and to ensure coherence across the PCN.

      Work with the PCN Clinical Directors and boards for strategic and operational planning processes to support the PCNs objectives

      Provide support to the Clinical Director and deputise in their absence.

      Arrange/ attend PCN, ICB and other stakeholder meetings and feedback the salient points.

      Work with clinical leads and other stakeholders as appropriate to develop a sustainable network.

      Develop an appropriate governance framework for the smooth operation of the PCN and the delivery of its plan. This will encompass the establishment of a legal framework between practices, agreed ways of working such as schedule of meetings, regular reporting on progress against the plan, and effective communications and engagement within and external to the network.

      Lead on PCN workforce development.

      Support recruitment, induction, and training of new PCN staff. Also manage the workflow processes.

      Monitor and evaluate performance of the Practice team against objectives; identify and manage change.

      Support practices QOF achievement when required via PCN projects.

      Co-ordinate the delivery of the PCN DES and related services.

      Lead on the administrative support for IIF achievements by interrogating, analysing, and maintaining the IIF data and interpreting the results.

      Communicate effectively at different levels within the practices both orally and in writing to help achieve the IIF targets.

      Work with practices within the network to identify areas of common concern or potential for collaborative working to improve the resilience of general practices, sustain local services and support recruitment and retention of key members of the primary care workforce.

      Effective use business intelligence systems / data sets to support the development of the PCN and integrated care. To interpret and analyse information which can be highly complex, sensitive and/or contentious.

      To produce reports on strategic/operational development to relevant parties to promote the work of the PCN and to account for progress.

      To analyse complex facts and information and interpret them for comparative purposes and for improving service delivery and/or project outcomes.

      Facilitate conversations to bring about changes across the system, based on good practice, patient experience or variation in performance, where appropriate, in a way that is supportive and non-judgemental.

      Arranging Super PPG meetings, including agenda and minutes.

      Maintaining PCN website and PCN Newsletter

      Build excellent relationships with partner organisations to both support the development of the network and the wider integrated care system.

      Lead in all activities required by CQC registration and compliance.

      Provide leadership and management skills to enable the PCN to meet its agreed aims and objectives and drive change within a profitable, efficient, safe and effective working environment leading to good CQC rating.

      Ensure a full understanding of PCN finances, making sure that adequate financial systems and reporting is in place.

      Monitor and check the accuracy of any submissions for Enhanced Services, Additional Roles Reimbursement Scheme (ARRS) and contractual requirements on behalf of the Network and ensure that all claims are submitted on a monthly and quarterly basis.

      Ensure that the PCNs physical and financial resources are managed and maintained for maximum effectiveness.

      Presenting financial plans, monitoring information and reports to the PCNs as required, managing any delegated budgets with propriety.

      Use knowledge of local and national strategies and frameworks to further develop the work of the networks, seeking opportunities to gain additional funding to enhance the scope and pace of development.

      Effective facilitation to drive forward the networks objectives and goals through meetings and workshops as required, create an inclusive environment, effectively setting guidelines and ensuring everyone has a voice to achieve the desired outcome in a timely manner.

      Ensure member practices are adequately supported with all work from Primary Care and that projects are effectively managed throughout their duration. This work will include reporting on specific milestones and outcomes.
