Job summary
Oakdale Park Primary Care Network (PCN) is a friendly, progressive and collaborative partnership between two practices, The Park Medical Practice and Overdale Medical Practice, serving areas including Borrowash, Breaston, Chaddesden, Oakwood and the University of Derby.
The PCN operates a multidisciplinary approach to healthcare, integrating a range of specialists to deliver high-quality, patient-centred care and addressing Health Inequalities in our community. Supported by GPs, Nurses and a range of other healthcare professionals, we work collaboratively and proactively reach out to develop new services for our patients in partnership with other Healthcare providers.
We offer a supportive environment where you will work alongside experienced professionals dedicated to mentorship and collaborative care. We will encourage personal growth in your chosen specialty and provide you with strong mentorship and peer support from day one.
We are extremely lucky that we have clinicians within our team who are registered trainers outside of the PCN, bringing their wealth and knowledge to our organisation.
We are excited to new role which is open to newly qualified GPs and is funded initially on a fixed term contract until 31.03.26
The 6 sessions week ( 4 sessions at Park Medical Practice and 2 sessions at Overdale Medical Practice)
Main duties of the job
The post-holder will manage caseloads and deal with a wide range of health needs in a primary care setting, ensuring highest standards of care for all patients.
Undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, issue repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems and screening patients for disease risk factors and early signs of illness.
Developing care plans for health in consultation with patients and in line with current practice disease management protocols and referral guidelines with clear and contemporaneous consultation notes to agreed standards, prescribing in accordance with the practices prescribing formulary.
The post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care with good understanding and experience of chronic disease management, and primary prevention and screening services.
Mandatory training must be up to date and compliant with all relevant practices policies and guidelines.
About us
The successful candidate will be working with the two practices, Park medical practice and Overdale medical practice.
You will be part of the Oakdale Park PCN team which is hosted on behalf of the PCN by Park Medical Practice.
The PCN is made up of 2 practices, so decision making is uncomplicated and we can move quickly when needed.
