      Job summary

      We are seeking an enthusiastic newly qualified GP to join our PCN. This will be a portfolio role which will involve both working within practices and supporting the development of frailty services in the local area.

      Working Hours: 4 sessions

      Main duties of the job

      Practice based work will focus upon

      • Holistic Assessments: Conduct assessments that consider the physical, emotional, and social aspects of patients' health, aiming to create personalised care plans.
      • Home Visits: Provide comprehensive medical care to patients in their homes, ensuring timely and effective treatment for those unable to visit the surgery.
      • Multi-disciplinary approach: work alongside other healthcare professionals, including nurses, social workers, and allied health professionals, to deliver integrated care.

      Frailty Service Development

      • work with colleagues and inform the design and implement services that address the needs of frail population
      • utilise data and analytics to understand population needs and work with colleagues to develop interventions
      • participate in quality improvement initiatives and contribute to the ongoing development of best practice within surgeries and wider PCN

      The successful candidate must be newly qualified, within 2 years, and not previously had a substantive post. You will be provided with the necessary training and development and CPD time within the scope of the role.

      About us

      Our South Southwark Primary Care Network (PCN) is made up of all 18 GP Practices in Camberwell, Peckham and Dulwich, approximately 156,000 patients, supported by their GP Federation, Improving Health (IHL). We are a large PCN with 60 staff in post to date and continue to expand over the coming year. We are committed to supporting and developing our staff, knowing this is crucial to our success. We have robust systems in place, including supervision and peer support, and continue to build on this as we expand our PCN team.

      Following the introduction of GP roles into the PCN Additional Roles Reimbursement Scheme (ARRS), we are looking for a newly qualified, enthusiastic, forward-thinking GP to join our PCN. This position offers an exciting opportunity to deliver high-quality care through both direct service delivery and contributing to the development of our frailty services. The successful candidate will work as part of a multidisciplinary team to enhance patient care and service delivery.

      Key Responsibilities include

      • Provide comprehensive and timely care to patients, ensuring appropriate diagnosis, treatment, and follow-up.
      • Undertake home visits for patients in need of medical attention, including those who are housebound or unable to attend surgery who have been referred by GPs.
      • Work closely with practice staff, community health teams, and the wider PCN to ensure continuity of care for patients across services.

      Experience of working in a primary care environment or similar healthcare setting

      Experience of dealing with a range of clinical conditions

      Experience of providing home visits

      Proven ability to evaluate the safety and effectiveness of own clinical practice.
