What to Expect (Job Responsibilities):
- Provide telephonic care and direction to members and facility staff during evening, overnight, weekend, and holiday hours
- Deliver acute care to assigned members efficiently and collaboratively with physicians
- Coordinate care as members transition through different levels of care and settings
- Ensure all quality elements are addressed and documented in accordance with regulations
- Regular communication with internal and external stakeholders to facilitate care coordination

What is Required (Qualifications):
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted license in the state of residence
- Active Nurse Practitioner certification through a national board
- 3+ years of clinical experience as an APC
- Proficient computer skills for documenting medical information

How to Stand Out (Preferred Qualifications):
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
- Understanding of Advanced Illness and end-of-life discussions
- Proven adaptability to change
