Job Description

- Coordination of physician appointments (as needed and required H&P schedule)
- Coordination of service plan resident reviews (Quarterly schedule).
- Resident satisfaction, clinical process audits—QAPI participation.
- Completion and compliance of Elopement Assessment.
- Completion and compliance of Health & Service Evaluation (HSE).
- Completion and compliance of Self-Administration Compliance Assessment.
- Regular audit (daily/weekly) shower sheets/stop & watch tool against shower schedule.
- Daily review of 24-hour report and follow up action (72-hour report on Mondays).
- Daily facilitation of mid-shift care huddle report.
- Assist with investigative measures for resident concerns/complaints as necessary.
- Daily occurrence report review and follow up, include notification requirement.
- Daily review of skilled treatment log (as applicable) and required action (ie: Q 7 day assessment, summary note, notification to family of depletion of 120 days)
- Daily reconciliation of new orders, medication/treatment changes (including refill needs), and corresponding family notification of both.
- Laboratory follow up—lab specimen collection, note in clinical record.
- Daily review of point of care completion compliance (task list).
- Weekly facilitation of resident supply list of needs (Tuesdays).
- As needed—timely completion of work order, resident apartment needs communication.

- LPN currently licensed in the State
- 2 years specific nursing experience working with a Dementia Population- Montessori Inspired Living training is a plus!
- Dementia Certification preferred
- At least 2 years of nursing experience required, Memory Care setting preferred
- At least 1 year of prior management experience
- Excellent verbal, written, and interpersonal communication skills

Friendship Village of Dublin is looking for an LPN Friend Leader to become our next Nurse Care Coordinator in our BRAND-NEW Memory Care Neighborhood!
30-40 hours per week (M-F), Rotating Weekends
Supervisor our warm, caring Nursing and STNA Friends provide oversight and assistance to residents from the moment they wake up to the moment the go to sleep, in a brand-new Memory Care Neighborhood (Single Site) that feels like Home!
This positions supervisors our nurses who are responsible for the supervision of all residents and the nursing assistants. They will organize, plan and direct assisted care for residents, and make daily assignments to nursing assistants.
