      What to Expect (Job Responsibilities):
      - Utilize clinical judgment and critical thinking skills for case management programs
      - Collaborate with healthcare team to develop individualized care plans
      - Monitor patient care remotely and in-person as required
      - Provide education, support, and materials to patients and clinic staff
      - Maintain accurate patient records and generate reports for management

      What is Required (Qualifications):
      - RN required with active unrestricted nursing licensure
      - 0-2 years of Nephrology Nursing and/or case management experience
      - Certified Case Manager (CCM), preferred
      - Strong written and verbal communication skills
      - Proficient computer skills, including Microsoft Office

      How to Stand Out (Preferred Qualifications):
      - Experience in patient education and/or case management
      - Knowledge of renal and diabetes disease processes
      - Leadership skills and ability to collaborate with clinic staff
      - Proficient in Microsoft PowerPoint
      - Ability to work independently and plan diverse activities
