Short Description
Nurse Navigator I - Bone and Joint
The Ambulatory Bone and Joint Nurse Navigator is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for Orthopedic and Rheumatology patients across several offices. The RN in this position is responsible for non-patient facing nursing care to include telephone triage, care coordination of complex conditions and partnering with provider colleagues in the provision of care excellence. Additionally, this RN serves as resource to LPNs in their provision of non-patient facing care.
Excellent customer service, interpersonal and clinical skills are required.
Experience in Orthopedics and/or Rheumatology preferred
Supports, educates and assists patients and their families, serves as a patient advocate. 25%
Demonstrates effective teamwork as a member of the interdisciplinary care team, fostering and exemplifying a culture of respect. May participate in the direct care of patients during the Provider visit. 25%
Responsible for the coordination of care, assists in securing prescriptions and supplies for proper care. Participates in telephone triage 25%
Consistently follows evidence based practice and nursing standards/procedures to provide nursing care that eliminates preventable events of patient harm and promotes excellent patient outcomes. 25%
