- Licensed Registered Nurse in the state or New Jersey.
- Board Certified Nurse Practitioner in the state of New Jersey.
- Masters or Doctorate prepared in Nursing from an accredited program, ACLS certified.
- Participates in the development of appropriate clinical protocols and related policies and procedures. Performs daily rounds.
- Supervising and coordinating clinical research trials within all phases of transplant, ensure continuity of care and effective patient recruitment for clinical studies, under guidance from the principal investigator.
- Collaborate with a multidisciplinary team and adhere to standards of safety and infection control in delivery of quality patient care.
- Consult with Infectious Disease Department to maintain current protocols for recipients and living donors
- Identifies need for family meetings for in-patients with complex clinical course, arranges transplant team members and works with Lead Transplant NP and attending physician or surgeon
- Evaluates pertinent diagnostic tests (including radiology, laboratory, ECG, echo, cardiac cath report) in accordance with NP protocol.
- Provides consultation, education, and referral services for transplant candidates, transplant recipients and living donors. This may include appropriate referrals to endocrinology (i.e. steroid induced diabetes); surgery (i.e. lymphocele drainage); cardiology (i.e. clearance for transplant candidate); urology (i.e. stent insertion/removal) etc.
- Provides consultation, education, and referral services for transplant candidates, transplant recipients and living donors.
- This may include appropriate referrals to endocrinology (i.e. steroid induced diabetes); surgery (i.e. lymphocele drainage); cardiology (i.e. clearance for transplant candidate); urology (i.e. stent insertion/removal) etc.
- Communicates with MD regarding pertinent abnormalities. Facilitates compliance of PRO-NJ requirements.
- Writes orders for pharmacologic agents/non-pharmacologic treatments (i.e. patient education, labs, etc.) for appropriate medical conditions according to NP protocol. Writes orders for Dialysis and plasmapheresis.
- Completes daily progress notes/discharge sheet in accordance with PRO-NJ requirements.
- Communicates with collaborating MD in a timely manner regarding pertinent patient issues.
- Assesses and manages health problems occurring during hospitalization per NP protocol.
- Performs clinical duties related to scope of practice including such things as staple removal; wound care; adjustment of immunosuppression; review of recorded BP and adjustment of anti-hypertensives accordingly; monitoring weight; adjusting diuretics/monitoring electrolytes etc.
- Educates patients and families regarding disease process, medical interventions and health promotion.
- Serves as a resource for nursing and ancillary staff about patient disease management.
- Acts as a liason between transplant coordinators, transplant physicians, transplant consultants and ancillary staff in providing continuity of care to patients and families
- Safely performs clinical procedures in addition to basic RN functions
- Facilitates scheduling procedures and arranging ancillary studies. Monitors patients during CTA testing.
- Handles medical problems and emergencies under supervision of attending. Documents in progress notes, writes orders and performs H&P if requested.
- Discusses plan of care with MD’s and multidisciplinary team.
- Performs daily rounds.
- Prepares presentations, participates in system meetings / external programs. Completes special projects as assigned.

Job Overview:
The Transplant NP practices as an independent care provider in collaboration with the transplant physicians, including the Chief of Transplant, in the inpatient Renal & Pancreas Transplant Unit, Transplant Short Stay Unit, Outpatient Pre and Post Transplant setting as well as Transplant Research supervising, coordinating clinical research trials.
Their practice is characterized by comprehensive health assessments, management of patients with acute and/or chronic renal disease. The NP shall have clinical responsibilities inclusive of transplant candidates, living donors, post-transplant patients and patients enrolled in clinical research trials.
Obtains patient histories and performs physical examinations. Writes admitting orders, routine orders and daily progress notes as indicated. Acts as liaison between Cooperman Barnabas and all referring dialysis centers, community agencies and referring physicians. Ensures continuity of care through effective communication and collaboration with the multi-disciplinary team in all areas of the transplant department. Uses an understanding of the principles of growth and development to assess each patient’s age specific needs and provides age specific treatment. Serves as a resource for nursing and ancillary staff in regards to patient and disease management. Facilitate practices that are in compliance with PRO-NJ and JCAHO requirements.
