      What to Expect (Job Responsibilities):
      - Conduct comprehensive assessments via Telehealth, including patient history and evaluation of data
      - Diagnose and manage acute and chronic conditions within established protocols
      - Develop and monitor individualized treatment plans, including prescribing medications and providing patient education
      - Manage administrative tasks associated with eClinicalWorks and respond to patient messages
      - Collaborate with healthcare providers to ensure high standards of integrated care

      What is Required (Qualifications):
      - Completion of a certified Physician Assistant program or accredited Nurse Practitioner program with state and national certification
      - Possession of a Physician Assistant license or Advanced Practice Registered Nurse license in good standing in Colorado
      - 2 to 5 years of experience as a Nurse Practitioner or Physician Assistant in primary care, internal medicine, or family medicine
      - Experience with Telehealth platforms and delivering virtual care
      - High proficiency with electronic health record systems

      - Ability to speak Spanish
      - Experience working with low-income and ethnic minority communities
      - Highly motivated, tech-savvy, and detail-oriented
      - Excellent interpersonal skills and ability to build rapport with patients and team members
