      What to Expect (Job Responsibilities):
      - Coordinate patient care delivery for a designated group of patients to achieve high-quality, cost-effective outcomes
      - Conduct daily calls to track, report, and monitor patients enrolled in the Remote Patient Monitoring program
      - Provide patient education and lifestyle coaching based on vital sign results
      - Collaborate with a multidisciplinary team to adjust patient care plans based on progress and feedback
      - Implement targeted interventions to avoid hospitalizations and emergency room visits

      What is Required (Qualifications):
      - Minimum of three years of Registered Nurse (RN) experience or equivalent relevant experience as a Licensed Practical Nurse (LPN)
      - Bachelor’s degree in nursing (BSN) or Associate’s degree in nursing (ADN) with 25 years of nursing experience
      - Currently licensed as a Registered Nurse in Nebraska or current compact state license
      - Basic Life Support (BLS) certification from an American Heart Association (AHA) endorsed program
      - Effective verbal and written communication skills and the ability to work collaboratively with an interdisciplinary team

      How to Stand Out (Preferred Qualifications):
      - Prior nursing experience in a related field
      - Master’s of Science in Nursing (MSN) degree
      - Experience with Microsoft Office applications, including Word and Excel
      - Knowledge of healthcare financial environments and reimbursement systems
      - Certification in relevant areas such as Advanced Cardiac Life Support (ACLS) or Certified Diabetes Educator (CDE)
