      Schedule: Per Visit

      Summary:

      As part of an inter-disciplinary team, the nurse, occupational therapist and handy worker engage with people in their home to reach the client-directed goals. The intervention is four months in length, with the RN providing monthly visits. The CAPABLE program was developed at the JH SON and training for this protocol-specific treatment will be provided. The RN assesses and brain-storms strategies to improve overall function and quality of life with each client, using motivational interviewing and assessing readiness to change.

      Education:

      B.S. or M.S. Occupational Therapy

      Required Licensure:

      Licensure from Maryland Board of Occupational Therapy required
