- Deliver acute, hospital-level care to patients in their homes using an evidence-based and patient-centered therapy approach.
- Help patients regain independence, improve daily function, and safely navigate their home setting, reducing hospital stays and improving outcomes.
- Work in collaboration with the interdisciplinary team to ensure that patients receive exemplary care in their home.

- Bachelor's Degree in Occupational Therapy Required
- 2 years prior relevant experience Preferred
- Licensure as an Occupational Therapist in the State of Wisconsin Upon Hire Required
- Basic Life Support (BLS) certification within 3 months Required

This position will work primarily in patient homes but has a physical work location at the Administrative Services Building in Madison, WI. Hours may vary based on the operational needs of the department.
