      Job summary

      This is an exciting opportunity to work in the Navigo Rehabilitation Team who are dedicated to supporting individuals in mental health rehabilitation, promoting empowerment and independence.

      We use a variety of approaches to enable service users to improve their independence, safety and quality of life. From working with individuals residing at Hope Court (community rehabilitation unit) and Brocklesby Lodge (inpatient rehabilitation) as well as in their own homes to provide practical and educational support and compensatory strategies to enable them to do what they need, want and like to do. We create plans with people to support them to achieve their goals and aspirations.

      Main duties of the job

      The successful applicant will directly line manage the rehabilitation Therapy team, as well as be directly involved in the assessment and delivery of identified occupational needs of the individuals using the service.

      You will be expected to:

      Hold a caseload in which you will be expected to manage autonomously and using prioritisation skills to ensure people are seen in a timely manner to appropriately manage any risks.

      Assess, develop, implement and evaluate OT care plans using a person centred approach with individuals using the service.

      Liaise and effectively communicate with all people involved in an individual's care, including consultants, AC/RC, social workers, nurses, Assistant Practitioners, carers and family members.

      Promote and develop innovative and creative ways of working using the resources available to achieve the possible care and outcomes.
