      Where you'll be working
      Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region.       What you'll be doing
      The Occupational Therapist (OT) role is an integral part of the Renal Supportive Care Multidisciplinary team providing holistic comprehensive assessment within the community/ and or hospital setting to a specialised group of patients with End Stage Kidney Disease requiring supportive care in their last weeks, months or years of life.

      The OT will develop measurable and realistic goals, care plans and priorities for timely intervention and implement and review the care plan with the team, client and/or carer and provide education and information to clients, carers and staff regarding ongoing intervention, equipment and community resources and ensure close liaison, communication and timely referrals to other team members and service providers.

      The Senior Occupational Therapist will demonstrate advanced reasoning skills and will operate autonomously with minimum direct clinical supervision providing care to the clients of the Renal Supportive Care (RSC) program. The OT will prioritise and monitor individual caseload to ensure timely access to services including SASH, My Aged Care, Enable and TACP.

      The Senior Occupational therapist will have advanced clinical reasoning skills and be able to apply these within home visits when providing recommendations for environmental modifications, energy conservation and equipment prescription.

      Essential Criteria:

      Qualification: Degree in Occupational Therapy
      Registration: Current Registration with Occupational Therapy Board of Australia (AHPRA).

      Selection Criteria:

      1. Demonstrated high level of clinical expertise and knowledge in the assessment and management of clients with complex conditions including experience in palliative care and/or chronic illness across community or hospital settings
      2. Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature
      3. Ability to provide clinical support, education and consultation to health professionals in area of specialty
      4. Demonstrated commitment to evidence based practice and quality improvement
      5. Excellent communication skills and ability to produce written documentation of a high standard
      6. Current driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD
