      Occupational Therapist (OT)- Pediatric Home Care 1099

      Queens, NY


      In order to be considered for this position, you MUST have a valid and active license in the state of NY. This is fee-for-service position, paid per visit and this is a 1099, contracted position.

      Job Overview: As an Occupational Therapist with Personal Touch Home Care, you will be responsible for assessing patients' needs, developing and implementing treatment plans, and helping clients improve their daily living and work skills. You will also provide rehabilitative services to patients who are unable to travel to a clinic or hospital. This is focused on improving patients' quality of life by promoting mobility, function, and independence within the comfort and convenience of their own homes.

      Key Responsibilities:

      1. Responsible for the assessment of referred patients that require rehabilitation services.

      2. Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

      3. Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.

      4. Set realistic and achievable goals for their patients.

      5. Document and record the patients’ condition.

      6. Educate patients and families in an appropriate therapy method.

      7. Evaluations, weekly progress notes, recertification and discharge documentation.

      8. Collaborates with all disciplines to provide the best patient centered treatment and care.

      9. Perform all other duties assigned by Director of Rehabilitation.

      10. Compliance with confidentiality, HIPPA and Federal Residents Rights.

      Qualifications:

      • Must have graduated from an accredited Occupational Therapist college or university.

      • Must have a valid professional license and registration for state of practice.

      • Experience with long-term care and/or geriatric care is preferred but not limited to.

      • Demonstrates good verbal and written communication and organization skills.

      • Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.

      Job Types: Fee-for-service, per diem

      Pay: $90.00 - $100.00 per visit
