- Conduct holistic assessments to identifyindividualneeds and develop person-centredsupportplans.
- Recommend home adaptations, equipment, and assistive technology to promote independence.
- Work collaboratively with multidisciplinary teams, carers, and family members to deliver high-quality interventions.
- Supporthospital discharge planning, preventing readmissions and enabling safe transitions into community settings.
- Promote strengths-based approaches that empower individuals toleadfulfilling lives.
- Provide education and training to staff, service users, and carers on occupational therapy interventions.
- Degree in Occupational Therapy and registration with HCPC.
- Experience working in a community or social care setting (desirable).
- Strong assessment skills with the ability to develop creative solutions to enhance independence.
- Excellent communication andinterpersonalskills for working with individuals, families, and professionals.
- Ability to manage a diverse caseload, balancing urgent needs and long-term interventions.
