      Job summary

      Are you an Occupational therapist or Physiotherapist wanting to work within the Mid Hampshire Area. The division is passionate about patient centred care, focusing on providing support and rehabilitation for both physical and mental health within the population's own environment. The teams are responsive, well led, and caring providing guidance to patients, relatives, carers and staff.

      We are calling for band 6 to apply for posts in our innovative Community Therapy Team. Community Therapy Teams are a supportive team of physiotherapists, occupational therapists, and support workers, providing domiciliary rehabilitation to the population of the Mid Hampshire. The Occupational Therapists will work with patients in their own environment with a functional focus to improve their quality of life and promote independence in the home and work.

      Main duties of the job

      The successful individual will be required to assess and develop patient centred treatment plans for a variety of conditions some will be very complex. They will be a great communicator, being able to talk to patients, carers and the wider multi-disciplinary team around care plans, and self management strategies.

      The individual will be required to link with the other health providers within the Mid and North patch to support with relationships between services, patient care and service development. They will support the team with their development and their own and take part in supervision for other members of the team and students.
