Job Description

- Responds to crisis calls in a timely manner
- Makes face to face visits when calls come into Access to Care or the Crisis Response Intensive Care Coordinator
- Provides services in the community, consumer’s homes, etc.
- Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
- Effectively determines the level of response needed in crisis situations
- Assists in contacting family members if needed
- Communicates effectively with Access to Care to assist in finding appropriate placement when necessary
- Evaluates the consumer for community referrals needed
- Educates families on de-escalating crisis situations, including but not limited to, safety plans
- Assists in securing emergency medical services or law enforcement if needed
- Advocates on behalf of the consumer to secure community resources
- Effectively collaborates with the treatment team members and outside sources in the delivery of services
- Follows up with consumers that are in AltaPointe Children’s Outpatient Programs who may be decompensating or at high-risk
- Travels to consumer’s homes to ensure medication compliance as needed
- Utilizes appropriate communication methods to ensure appropriate communication with individual consumers’ treatment teams
- Manages and triages calls from multiple referral sources
- Effectively demonstrates the application of adult specific competencies
- Provides transportation to services on a limited basis
- Seeks clinical supervision and consultation as needed
- Accepts and employs suggestions for improvement
- Actively works to enhance case management skills
- Documents in a timely fashion per AltaPointe policy
- Documents using Avatar appropriately
- Documents appropriate, complete, and comprehensive assessments
- Documents in detail the resolution of crisis situations and linkage to appropriate follow up sources
- Documents in a clear and concise manner services provided before the completion of the shift
- Documents legibly
- Effectively manages caseload based on consumer needs
- Meets AltaPointe productivity standards if applicable
- Knowledge of psychiatric disorders in the child/adolescent population
- Knowledge of cultural diversity
- Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practice
- Knowledge of Psychiatric Rehabilitation principles
- Knowledge of available community resources
- Knowledge of educational methods appropriate to children/adolescents with psychiatric disorders
- Knowledge of substance abuse
- Knowledge of AltaPointe Health Systems programs and services

Qualifications
Bachelor’s degree in a mental health discipline, or a nurse, or a certified parent peer specialist with lived experience working with crisis situations.
Must be at least age 23, maintain a valid verifiable driver license and maintain a clear driving record.
Knowledge in child/adolescent specialty, outlined below, to be attained within (6) six months of employment:
