What to Expect (Job Responsibilities):
- Engage individuals with chronic health conditions through various methods to enroll them in the program
- Conduct progressive outreach attempts for referred and pending clients on a weekly basis
- Schedule and conduct intakes to complete consent forms and gather necessary information for enrollment
- Maintain documentation in the Electronic Health Record (EHR) in a timely manner
- Travel to Nassau and Suffolk to participate in community events to promote the program

What is Required (Qualifications):
- Associate's Degree in the human services field or equivalent plus one year of related experience
- Professional appearance, excellent organizational and time management skills, strong communication and public speaking skills
- Valid NYS Driver’s License and access to an automobile
- Proficiency in computer applications such as Microsoft Office
- Ability to work independently and collaboratively with team members

- Bachelor’s Degree in the human services field
- Previous experience in a related field
- Strong interpersonal skills and ability to work with diverse populations
