      Outreach/ Intake Worker

      EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Outreach/ Intake Worker to work part time for its Long Island (Nassau County) Health Home program.

      The Outreach/ Intake Worker position is a non-exempt position paying $18-22 per hour. Hybrid schedule of Monday-Friday 9AM-5PM. The office is located in Hempstead, NY. The role reports to the Program Director.

      Primary Purpose of Job:

      The Outreach/Intake Worker conducts outreach in order to engage individuals both within the organization and within the community for the purpose of assessing their qualifications for enrollment. Responsible for enrollment into the program, including checking Medicaid status. Responsible for promoting program at various events, both virtual and in person.

      Principal Duties & Responsibilities:

      1. Engage individuals with chronic health conditions (mental health, substance use, and/or medical) through various methods (phone, face to face, etc.) in order to enroll them into the program.
      2. Conduct progressive outreach attempts for all referred and pending clients at a minimum on a weekly basis.
      3. Schedule and conduct intakes to complete consent forms and gather necessary information in order to enroll clients into the program.
      4. Maintain all necessary documentation in the Electronic Health Record (EHR) in a timely manner.
      5. Monitor Medicaid/MCO status for all clients in outreach status at the beginning of each month to ensure continued eligibility.
      6. Work collaboratively with Care Coordination Director and other team members regarding any client needs.
      7. Participate in supervision to address concerns/issues and improve skill development.
      8. Travel to Nassau and Suffolk and participate in community events to promote the program to potential eligible clients.

      Knowledge, Skills & Abilities Required:

      1. Bachelor’s Degree in the human services field preferred or Associate’s Degree in the human services field or equivalent plus one year of related experience.
      2. Must have professional appearance, excellent organizational and time management skills, strong communication and public speaking skills and the ability to work independently.
      3. Must have valid NYS Driver’s License and access to automobile
      4. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
