What to Expect (Job Responsibilities):
- Conduct direct telephonic outreach to patients on the provided list
- Assist patients in setting up appointments with a member of the clinical care team in partnership with their primary care provider (PCP)
- Apply motivational interviewing tactics to address psychosocial barriers to scheduling appointments
- Educate patients on services offered by the practice(s) and respond to their questions and concerns
- Track outreach and outcome documentation in the designated tracking system

What is Required (Qualifications):
- High School Diploma
- Minimum of 2 years of healthcare experience
- At least 1 year of telephonic customer service experience
- Understanding of call center performance operations
- Must have access to a reliable high-speed internet connection

How to Stand Out (Preferred Qualifications):
- Experience working in a medical practice or healthcare setting
- Call center experience; bilingual skills are preferred
- Medical Terminology certification
- Proficiency with Microsoft and Google applications
