      The San Francisco Homeless Outreach Team (SFHOT) is an exciting program that has been awarded for its continued dedication in serving the homeless. SFHOT is comprised of a multi-disciplinary staff including a street medicine team, shelter health team, case management program street outreach workers, with licensed clinicians providing clinical oversight for the team.

      The Outreach Supervisor works closely with a team of other licensed clinicians to provide clinical supervision and administrative coordination for a team of outreach workers who outreach multi-diagnosed, homeless individuals throughout the city of San Francisco. Under the supervision of the Program Supervisor and the Chief Program Officer, the Outreach Supervisor provides leadership in overseeing client outcomes i.e., benefits, health care, permanent housing, while providing quality assurance and operational oversight.

      The Outreach Supervisor functions within a team composed of staffing from the Department of Homelessness and Supportive Housing’s (DHSH) and Heluna Health, a private non-profit, and interacts with other City Departments, non-profit and for-profit organizations. The Outreach Supervisor’s work occurs within the context of HSH’s clinical policies including Harm Reduction and the Recovery Philosophy.

      ESSENTIAL FUNCTIONS

      • Works as part of a team to provides clinical and administrative oversite for outreach workers.
      • Responds to team requests for support, especially around crisis intervention; provides effective de-escalation and crisis intervention including placement of 5150’s, evaluations for DTS/DTO/GD, stabilizes clients who are homeless, provides home visits in SRO’s, street environments, shelters, treatment facilities and jail/prison as needed; works frequently in the public eye, occasionally under media scrutiny.
      • Provides weekly individual and group clinical supervision for staff members/team that focuses on clinical support, creating a wellness and recovery service environment, resolution in life skills, and facilitating beginning and advanced clinical trainings.
      • Establishes positive relationships with clients, staff and other providers in community; fosters cooperative work environment. Assists staff and clients with explanation of diagnosis and service plan development, prepares notes for individual cases which may include diagnostic assessment and treatment records, such as therapy notes.
      • Facilitates team meetings as appropriate, typically weekly. Attends all other meetings as required.
      • Maintains records of all activities relating to homeless outreach, prepares periodic reports as needed, confers with supervisors and management steam on formulation of policies and procedures; attends case conferences with other clinics and agencies. May also provide other duties as assigned.
      • Acts as On-Call Supervisor, some night and weekend on-call shifts may be required.
      • Other duties as assigned.

      MINIMUM QUALIFICATIONS

      1. Preferred Master’s degree in social work with one year of experience as a psychiatric social worker or medical worker in an inpatient or outpatient facility, or in case work or outreach with homeless people.
      2. Previous managerial experience.
      3. Must possess a valid California Divers License.

      AND

      1. Valid California driver’s license and no history of major moving violations, including DUIs, during the past 5 years.
      2. Advanced level of knowledge of the client population and their complex needs including homelessness, financial instability, medical and mental health, and substance abuse.
      3. Ability to work well with diverse staff and clientele including cultural, language, sexual identity, gender and other diversity considerations within all neighborhoods in San Francisco.
      4. Ability to work successfully both independently and cooperatively.
      5. Ability to speak read and write clearly in English.
      6. Advanced level of competency using Internet, email, and Microsoft Word computer applications as well as ability and willingness to learn necessary programs needed for everyday job function.
      7. Ability to use computers/internet to look up information and enter relevant notes.
      8. CPR and first aid certification within 90 days of hire.

      PREFERRED QUALIFICATIONS

      • Experience in a community-based setting serving ethnically diverse, low-income clients. Knowledge of community resources and experience in community settings.
      • Conflict resolution skills
      • Customer service orientation.
      • Analytical ability and skills to handle and solve complex organizational and clinical issues and problems.
      • Computer knowledge/skills or ability to learn.
      • Excellent oral and written communication skills.
      • Language capability: Spanish, Cantonese, Mandarin, Tagalog.

      PHYSICAL DEMANDS

      • Must be capable of performing job duties requiring frequent use of the entire body including ability to stand, walk, climb stairs, sit, drive for extended periods of time, exit and enter vehicles throughout your work shift.
      • Ability to use a computer, phone or office equipment for extended periods of time.
      • Ability to successfully and efficiently complete tasks in an environment where background noise is present, and interruptions may be constant.
      • Must be able to lift a minimum of 35 lbs.
      • With instruction, the ability to assist persons with disabilities and to help transfer a 180 lb person and lift wheelchair in and out of a car or van.
