      The San Francisco Homeless Outreach Team (SFHOT) is an exciting program that has been awarded for its continued dedication in serving the unhoused population. SFHOT is comprised of a multi-disciplinary team including case management, street outreach, encampment resolution, and street crisis response team. They also have a team of licensed clinicians providing clinical oversight for the team.

      The Outreach Supervisor works closely with the SFHOT Program Manager and the Department of Homelessness and Supportive Housing staff to provide supervision and administrative coordination for a team of outreach workers who outreach multi-diagnosed, unhoused individuals throughout the city of San Francisco. Under the supervision of the Program Manager and the Chief Program Officer, the Outreach Supervisor provides leadership in overseeing SFHOT’s provision of services to the unhoused population of San Francisco i.e. One system profile creation, assessments, benefits, health care, permanent housing, while providing quality assurance and operational oversight.

      The Outreach Supervisor functions within a team composed of staffing from The Department of Homelessness and Supportive Housing’s (HSH) and Heluna Health, a private non-profit and interact with other City Departments, non-profit and for-profit organizations. The Outreach Supervisor’s work occurs within the context of HSH’s clinical policies including Harm Reduction and the Recovery Philosophy.

      ESSENTIAL FUNCTIONS

      • Works as part of a team to provides oversite for outreach workers.
      • Responds to team requests for support, around crisis intervention; provides effective de-escalation, stabilizes clients who are homeless, provides home visits in SRO’s, street environments, shelters, treatment facilities and jail/prison as needed; works frequently in the public eye, occasionally under media scrutiny.
      • Provides individual supervision for staff members/team that focuses on client support, creating a wellness and recovery service environment, resolution in life skills, and facilitating beginning and advanced trainings.
      • Establishes positive relationships with clients, staff, and other providers in the community; fosters a cooperative work environment.
      • Assists staff and clients with explanation of diagnosis and service plan development, prepares notes for individual cases.
      • Facilitates team meetings as appropriate, typically weekly. Attends all other meetings as required.
      • Mentor and train staff on developing proper outreach skills, which include directing to follow proper policies and procedures.
      • Maintains records of all activities relating to homeless outreach, prepares periodic reports as needed, confers with supervisors and management team on formulation of policies and procedures; attends case conferences with other clinics and agencies.
      • Acts as On-Call Supervisor, some night and weekend on-call shifts may be required.
      • Other duties as assigned.

      MINIMUM QUALIFICATIONS

      1. Preferred degree in social work with at least one year of experience in case work or outreach with homeless people.
      2. Previous managerial experience, directly supervising other employees.
      3. Must possess a valid California Divers License.

      AND

      1. Valid California driver’s license. Driver must be 21+ with none of the following in the past 3 years: Any 2-point convictions; or more than (2) moving violations, or more than (1) moving violation and (1) at-fault or passenger-handling accident.
      2. Advanced level of knowledge of the client population and their complex needs including homelessness, financial instability, medical and mental health, and substance abuse.
      3. Ability to work well with diverse staff and clientele including cultural, language, sexual identity, gender and other diversity considerations within all neighborhoods in San Francisco.
      4. Ability to work successfully both independently and cooperatively.
      5. Ability to speak read and write clearly in English.
      6. Advanced level of competency using Internet, email, and Microsoft Word computer applications as well as ability and willingness to learn necessary programs needed for everyday job function.
      7. Ability to use computers/internet to look up information and enter relevant notes.
      8. CPR and first aid certification within 90 days of hire.

      PREFERRED QUALIFICATIONS

      • Experience in a community-based setting serving ethnically diverse, low-income clients. Knowledge of community resources and experience in community settings.
      • Conflict resolution skills
      • Customer service orientation.
      • Analytical ability and skills to handle and solve complex organizational and clinical issues and problems.
      • Computer knowledge/skills or ability to learn.
      • Excellent oral and written communication skills.
      • Language capability: Spanish, Cantonese, Mandarin, Tagalog.

      PHYSICAL DEMANDS

      1. Must be in good physical condition and capable of performing job duties requiring frequent use of the entire body including ability to stand, walk, climb stairs, sit, drive for extended periods of time, exit and enter vehicles throughout your work shift.
      2. Ability to use a computer, phone or office equipment for extended periods of time.
      3. Ability to successfully and efficiently complete tasks in an environment where background noise is present and interruptions may be constant.
      4. Must be able to lift a minimum of 35 lbs.
      5. With instruction, the ability to assist persons with disabilities and to help transfer a 180 lb person and lift wheel chair in and out of a car or van.
