- Delivers direct services to program participants. This includes assessment, planning, respite and mentor services, and referral information, as required by the program.
- Delivers comprehensive services consistent with the program plan, as required by the program.
- Delivers supportive services to program participants which may include transportation.
- May attend community events with program participants.
- Works under the direct guidance and supervision of an assigned staff member.
- Documents all services provided according to program guidelines and may complete monthly or quarterly reporting or billing.
- High School Diploma/equivalent required. Some college preferred.
- At least one year of relevant experience.
- Must be at least 21 years of age.
- Availability to work evenings and occasionally on weekends.
- Professional, effective oral and written communication style required.
- Well organized, detail-oriented, resourceful, and self-directed.
- Valid driver’s license, auto insurance, and reliable transportation required.
- Compensation: Hourly Range $16.60-$18/hour; offers are commensurate with experience and bilingual candidates may receive additional compensation. The salary range provided represents our current estimate for this role at the time of posting and is subject to change. The final salary will be determined based on a range of factors, including but not limited to, a candidate's relevant experience, education, qualifications, skills, certifications, and alignment with organizational needs. - Location: services are offered in the 9 surrounding Northern counties (Stephenson, JoDavies, Boone, Winnebago, Lee, Ogle, Dekalb, Whiteside, Carroll) to families in their homes and other community-based locations as well as virtually when necessary; between in-person client services, staff may work from the Regional Rockford Office (424 7th Street) or a remote location.
- Schedule: Part-time, hourly; flexible scheduling includes some evening and weekend appointments.

The Outreach Worker is a critical member of the Adoption Support & Preservation team, meeting with children and teens to offer mentoring and opportunities for positive youth development activities. The Outreach Worker also provides parents with applicable community referrals. As an Outreach Worker, you will provide one-on-one respite and mentoring services to adoptive children with behavioral and attachment challenges, work with program therapists to implement goals, plan group activities, facilitate monthly children’s social groups, and refer parents to needed community resources. This flexible part-time position includes some evening and weekend hours and is a great way to start your career in the social work field.
