      PACE Partners of Northeast Florida, also known as PACE (Program for All-Inclusive Care for the Elderly), is a joint venture between Community Hospice & Palliative Care and Aging True. Care coordination for the program, the first of its kind in North Florida, is delivered at our beautiful day center known as The PACE Place.

      Our clinic and adult day center are located on the Westside of Jacksonville, next door to the Acosta-Rua Center for Caring.       Under the supervision of the PACE Center Manager, the PACE Transportation & Schedule Coordinator is responsible for the PACE Partners Transportation program, including transportation and schedule coordination of participants, transportation of meals, delivery and pick-up of durable medical equipment, and medication; transportation scheduling; oversight, hiring, training and supervision of contracted drivers and dispatchers and any other transportation as deemed necessary. Responsible for meeting departmental budget goals, maintaining good communication with transportation vendors, and maintaining appropriate records of each driver to ensure proper coordination of services.

      Duties and Responsibilities :

      • Supervise the transportation departmental staff

      • Responsible for departmental hiring, orientation, competency, and training procedures

      • Participate in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions, and coordinating care delivery

      • Communicate interdisciplinary team decisions, participant changes, care plan changes, and updated information to transportation staff on a daily basis

      • Ensure responsibility for running and maintaining the transportation scheduling software program

      • Ensure effective daily communication between PACE Partners staff, transportation drivers, and dispatch

      • Ensure the scheduling, coordination, and communication of the driver route assignments of PACE Partners and assure the daily work schedule is communicated

      • Ensure the scheduling of transportation for all participants outside appointments

      • Monitor use of driver/dispatch overtime, ensure adequate staffing and improve efficiencies

      • Coordinate and schedule the daily delivery of home-delivered meals, durable medical equipment (DME), supplies, and miscellaneous items as requested by the IDT team

      • Manage and update the participant database

      • Oversee vehicle maintenance program and obtain estimates and authorize vehicle repairs per departmental policies

      • Research new vehicle options and assist in the ordering and purchasing of new vehicles

      • Ensure that vehicles have up-to-date Department of Motor Vehicle registration forms, license plates, and emission stickers

      • Ensure cleanliness and proper functioning equipment as well as safe driving conditions for each vehicle

      • Ensure that all departmental staff attend and successfully execute required annual driver training courses and participate in mandatory in-service training

      • Set clear expectations and assign appropriate authority to staff; hold staff accountable for their scope of work and compliance with company policies and procedures

      • Follows all Policies and Procedures and OSHA safety guidelines

      • Maintain the confidentiality of all company procedures, and information about participants, clients, or families

      • Handle potentially infectious specimens with appropriate biohazard precautions, and practice standard precautions

      • Participate in and supports Quality Improvement initiatives

      • Participate in continuing education classes and any required staff and training meetings Maintain professional affiliations and any required certifications

      • Perform other duties as assigned

      Qualifications and Requirements:

      Education/Training/Certifications

      • High School diploma or GED

      • Valid Florida driver’s license

      • Active Auto Insurance

        • Must have, or obtain upon hire, First Aid and CPR certification

        • Bi-lingual Spanish preferred

      Experience

      • 2 years of administrative/supervisory experience, in a transportation setting

      • 5 years of experience in the transportation industry preferred

      • Experience working with contractors and vendors

      • Minimum of 1-year experience transporting the frail and elderly

      • AHCA Level II background check

      Skills and Knowledge:

      • Knowledge of safety requirements pertaining to the transportation industry

      • Requires an individual who can coordinate multiple tasks simultaneously

      • Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and the public

      • Proficient in identifying and recommending problem resolution

      • Ability to work independently with minimum supervision

      • Excellent verbal and written communication skills

      • Must have a good working knowledge of MS Office Suite including Word, Excel, PowerPoint, and Outlook. This position utilizes MS Excel often and must have strong knowledge of spreadsheets and formulas

      • Ability to manage a transportation scheduling computer software

      • Familiar with operation of accessible vehicles

      • Experience with maintenance and oversight of a fleet of vehicles
