Job summary
We are pleased to introduce an exciting new role, funded as part of the Paediatric Critical Care (PCC) Level 2 designation for Wexham Park Hospital. This post sits within the Paediatric Directorate at Frimley Health NHS Foundation Trust (FHFT) and plays a vital role across the Integrated Care System (ICS), supporting children and young people requiring step-down care from PCC Level 3 and tertiary Level 2 services into our designated Level 2 unit. The post holder will lead and coordinate discharge planning from the point of admission, ensuring smooth and safe transitions of care.
This role aims to enhance patient flow through effective discharge strategies, reduce unnecessary hospital admissions, and act as a central point of contact for children and young people (CYP), families, and carers--particularly those with complex needs or extended stays. The post holder will work closely with multidisciplinary teams across acute and community settings, supporting family-centred care, continuity, and communication. This is a unique opportunity to contribute to the development of the PCC pathway and deliver high-quality, integrated care for our patients.
Main duties of the job
The post holder will lead the coordination and facilitation of safe, effective discharge planning for children and young people within the paediatric directorate at Frimley Health NHS Foundation Trust (FHFT). This includes discharge to home, local hospitals, and community services. Working collaboratively with the multidisciplinary team, the role ensures timely discharge by identifying barriers early and liaising with internal and external stakeholders. The post holder will act as the central point of contact for discharge planning, attending ward rounds, family meetings, and case conferences as needed. They will liaise with community nursing, social care, education, and other relevant agencies to ensure seamless transitions of care. The role includes maintaining accurate documentation, ensuring care plans and referrals are in place prior to discharge, and supporting families through education and information-sharing. They will contribute to service development by identifying areas for improvement in discharge processes and supporting audit and quality improvement initiatives.
