- As a key member of a multi professional team, ensure appropriate referral anddiscussion with other members of the team to optimise patient care.
- Ensure patients are assessed, have care plans that are implemented andevaluated in accordance with Trust's evidence based procedural documents.
- To have local open and transparent communication channels that support themultidisciplinary team to do the right thing for people we care for, to be bold whenthey have good ideas and to speak up when things go wrong.
- Act as a clinical expert for palliative and end of life care.
- Ensure staffing is in line with Trust policy.
- Manage budgets and resources efficiently and effectively.
- Champion a culture of learning and quality improvement within the team
- Be an ambassador for palliative and end of life care across the Trust.
- Appropriate NMC registration (Adult Nursing)
- Degree level study or willingness to work towards Palliative care qualification
- Mentorship/ Teaching qualification
- Advanced Communication course or similar
- Master's degree or working towards in relevant subject
- Advanced physical assessment qualification
- Non-medical prescriber, or ready and willing to train as such
- Extensive experience within Palliative/ end of life care/long term conditions
- Experience of managing a clinical caseload.
- Experience of delivering educational sessions and developing training programmes
- Ability to deal with difficult emotional situations
- Recruitment and selection skills
- Evidence of leadership skills
- Experience with managing change in the work place
- Assertiveness skills

Job summary
An exciting opportunity has arisen for a Band 7 Macmillan Palliative care Clinical Nurse Specialist to join our dynamic team Imperial College Healthcare.
As a Palliative care Clinical Nurse Specialist you will provide expert palliative care nursing advice, to support and facilitate high quality, individualised care to patients and relatives/carers, in collaboration with the multi professional team, in line with the local and national targets.
The successful candidate willmanage their own caseload of patients, working as a part of the multi-disciplinary Specialist Palliative Care Team. They will maintain effective communication with patients, families/carers and other healthcare professionals, to ensure seamless service delivery.
As part of the Specialist Palliative care team, you will undertake comprehensive palliative care assessments identifying/diagnosing patients physical, social, psychological, cultural and spiritual needs; to facilitate the provision of quality nursing care and to contribute to decisions regarding clinical care and priorities.
