      Job Description

      Knowledge of advanced directive and ability to assist patients and families with completion when needed

       Able to work independently and as part of a multidisciplinary team

       Knowledge and skills to screen patients for palliative care needs

       Ensure effective communication is established with the patients, caregivers, families, and all disciplines to facilitate a successful transition across the continuum of care

       The ability to ensure the confidentiality and rights of all patient and departmental documents is required

       The ability to effectively communicate and teach stages of chronic conditions

       The ability to effectively communicate end of life care and make referrals to Hospice care when needed

      Qualifications

      To qualify for the position, the candidate must have a current licensure as a registered nurse. The candidate must have at least two years’ experience in a hospital setting. The candidate is preferred to have a BSN or other related bachelor level degree. Certification in palliative care not mandatory to start, yet requested to be completed within 2 years of employment. An authentic belief and acceptance of Palliative Care principles is essential.
