      JOB SUMMARY

The Palliative Medicine Physician will work within an interdisciplinary team to provide inpatient consultation to patients with serious illness who are seeking assistance with advanced symptom management, clarifying goals of care, addressing end of life issues and care, and transition planning. Consults will be provided to all types of admissions including medical, surgical, and ICU and will augment care provided by the attending hospitalist or surgical team. Our Palliative Medicine Physicians work directly with the patient and their families and caregivers, and their hospital and specialty providers, to create a customized holistic plan of care. Our goal is to improve patient and caregiver quality of life on their terms and optimize service by aligning clinical care with patient values and goals.

As part of a growing service line, the Palliative Medicine Physician will be a key partner in shaping the care model through active participation in process and quality improvement as well as embracing creative process. The physician will play a lead role in fostering a culture of collaboration and a high functioning team within the Palliative Care Service. Additionally, the Palliative Medicine Physician will be an ambassador to colleagues in promoting awareness, knowledge, and skill in delivering palliative care throughout the medical group.

Our team is dedicated to providing comprehensive patient care wherever it’s needed, ensuring seamless coverage across various settings. While most doctors operate from a single location, our small, collaborative team engages in cross-coverage through virtual work and may also support other outpatient sites or in hospital to meet patient needs and assist fellow teammates.
