Job summary
We are looking to recruit a paramedic with 5 years of experience to work as part of a multi-disciplinary team, in our new urgent care centre which will be in Bursledon.
Paramedics are employed by the Primary Care Network to support population health management through on-the-day demand and access with home visits, treatment of minor ailments and injuries and other support relevant to General Practice.
Main duties of the job
To be part of a multi-skilled primary care team providing patient centred medical services based on patient needs with a focus on continuity of care, including,
Working alongside the team within the primary care network to provide a first point of contact for patients presenting with medical problems at home, making use of skills in history taking, physical examination, problem solving and clinical decision-making, to establish a diagnosis and patient centred management plan.

Over the past five years, our two practices have worked closer together by way of the Primary Care Network (PCN). Lead certain community services i.e. monitoring blood pressure and diabetes risk for elderly patients.
Perform urgent and same-day home visits.
This role will work as part of the PCN MDT, working collaboratively with primary care network staff to promote integrated and seamless pathways of care. Instigate necessary investigation/non-invasive diagnostic tests/investigations and interpret findings/reports.
Refer patients to an alternative care setting or treat and discharge as appropriate.
Contribute to the locality practices achieving its quality and quantitative targets to sustain the high standards of patient care and service delivery.
Help to develop and set up new patient services and participate in initiatives to improve existing patient services.
