      Job summary

      We are looking to recruit a paramedic with 5 years of experience to work as part of a multi-disciplinary team, in our new urgent care centre which will be in Bursledon.

      Paramedics are employed by the Primary Care Network to support population health management through on-the-day demand and access with home visits, treatment of minor ailments and injuries and other support relevant to General Practice.

      Main duties of the job

      To be part of a multi-skilled primary care team providing patient centred medical services based on patient needs with a focus on continuity of care, including,

      Working alongside the team within the primary care network to provide a first point of contact for patients presenting with medical problems at home, making use of skills in history taking, physical examination, problem solving and clinical decision-making, to establish a diagnosis and patient centred management plan.

      About us

      The primary care network consists of Blackthorn Health Centre and Hedge End Medical Centre. Over the past five years, our two practices have worked closer together by way of the Primary Care Network (PCN). Together, we have enjoyed developing and offering new services and collaborating on successful projects to provide better care for our patients.

      This partnership is a significant step forward for both practices and our patient community. It represents, not just a merging of resources, but a fusion of our vision to provide a responsive, innovative and flexible service for our patients. It will also help safeguard GP services for many years to come.

      The merging of two established and resilient practices provide the opportunity to create a new organisation on a long term sustainable footing able to capitalise for patients the strengths and opportunities for improvement that come with scale whilst still supporting the fundamentals of general practice so valued by patients and the GP partners.

      Lead certain community services i.e. monitoring blood pressure and diabetes risk for elderly patients.

      Perform urgent and same-day home visits.

      Essential

      • -In total 5 years previous experience in a paramedic role.
      • -The post holder will be expected to work with a high degree of independence and manage their own workload according to priorities.
      • -To establish effective networks with internal and external stakeholders.
      • -Demonstrable patient assessment skills.
      • -Knowledge of general practice related medical conditions, treatments and pharmacology.
      • -Knowledge of maintaining confidentiality and implementing data protection legislation requirements.
      • -Knowledge of safeguarding principles.
      • -Ability to work as a team member and autonomously within own scope of practice, understanding when to refer to GPs.
      • -UK Driving Licence and own vehicle.

      Qualifications

      • Health & Care Professionals Council registration.
      • Registered paramedic with 5 years post registration experience.
      • Excellent communication skills.

      Desirable

      • Ability to use EMIS medical system

      • Following agreed clinical protocols with referral to senior healthcare professionals or GPs as appropriate.
