- Post registration Parkinson's qualification or equivalent experience
- Teaching/Mentorship qualification or equivalent experience
- Possession of a degree or equivalent
- Hold current registration as an Independent non-medical prescriber with the NMC.
- Experience of working with Parkinson's patients
- Experience of working within a community setting.
- Demonstrates ability to develop new initiatives.
- Demonstrates ability to managing change
- Understanding of the management of patients with Parkinson's Disease
- Ability to communicate effectively, verbally and in writing.

- Undertake NMP training.

- Computer skills i.e. Word, data- bases email, internet
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)Friendly and approachable.

Job summary
To develop and deliver a specialist community-based service for patients with Parkinson's Disease (PD), acting as an expert resource in PD by working closely with patients, carers, consultants, general practitioners, therapists, social services and other stakeholders across primary and secondary care.
Aim to minimise the effect of PD on patients' lives by providing crisis intervention, stabilising conditions, preventing unnecessary A&E attendances and hospital admissions, and decreasing length of stay.
Work with the specialist teams to develop and implement a NICE guidance compliant service that supports multi-disciplinary teams caring for PD patients by providing education and training to multi-disciplinary teams in the best practice management of PD. To work with the Service Management Team in providing leadership management of Parkinson's Disease Services
Main duties of the job
To work autonomously to deliver Parkinson's specialist clinical care appropriate to the needs of the patient group and based on principles that are evidence-based and which enhance the quality of patient care.
To undertake comprehensive assessments of complex Parkinson's patients with co-morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning.
To act as an autonomous and specialist practitioner and be responsible for leading and managing a clinical caseload of patients with Parkinson's disease and advise and support the wider MDT.
Manage a caseload of patients with varying PD problems, providing patients and families with information on secondary prevention, education, advice and encouragement, maintain and record accurate documentation for all patients.
To work in a variety of clinical areas, including patients' homes and outpatient clinics.
To work as an autonomous practitioner accepting clinical responsibility for a specialised caseload of patients and to organise this effectively and efficiently about clinical priorities and use of time.
