Job Description

- Provide crisis stabilization services to individuals experiencing a mental health crisis while they are in the Crisis Residential Units
- Manage distraught or psychotic clients who are in need of structure; monitor gross psychological and physical functions of clients
- Familiarize clients with guidelines and procedures of the residential units
- Meet with clients to discuss individual plans, living arrangements or any difficulties they may be having
- Identify services needs and develop treatment or referral services plan with clients.
- Perform functional assessment of client to determine client’s strengths and abilities; assist clients in achieving their short term goals
- Identify and secure in-home services to maintain stabilization
- Contact service providers to request and arrange services; coordinate and monitor services rendered
- Confer with client caregivers, families, physicians, lawyers, community service providers and other professionals throughout the County
- Assist clients to maintain basic needs
- Provide outreach, screening and engagement services
- Respond to the immediate needs of referrals
- Arrange for the discharge of inpatient referrals and coordination of transition to the community
- Provide outreach services to reengage consumers who have relapsed or are nonresponsive.
- Provide Targeted Case Management (TCM) for assigned consumers to aid in achieving goals identified in the individualized treatment plan.
- Provide Community Psychiatric Supportive Treatment (CPST) for assigned consumers to achieve treatment goals.
- Provide Attendant Care (AC) for assigned consumers to achieve treatment goals.
- Provide transportation to support client attendance in medical appointments and other therapeutic goal related activities
- Prepare quality treatment plans in collaboration with client and/or guardian, additional treatment team and community partners.
- Facilitate treatment plan review.
- Complete DLA-20 at initial case management appointment and quarterly thereafter.
- Complete progress notes in accordance with COMCARE and CCBHC guidelines
- Complete Notes for Record, releases of information, safety documents and other documentation as required and/or recommended within established deadlines.
- Completes court reports or treatment summaries within required timelines.
- Ensure client’s financial information is current in medical record.

Will maintain a caseload of individuals that are high utilizers of emergency services. Integrated Care Specialist will provide services to individuals that are currently involved with KDOC, or being released from inpatient psychiatric hospitalization. They will be able to maintain a schedule while also providing services for crisis related needs.

Minimum Qualifications: Bachelor’s Degree or equivalent education and experience. If no degree is present applicant may substitute each one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, SRS child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
Preferred Qualifications: One year of experience in a Community Mental Health Center and/or supervisory experience.
