What to Expect (Job Responsibilities):
- Conduct virtual assessments and monitor patients' conditions remotely
- Develop and coordinate individualized care plans in collaboration with the interdisciplinary team
- Provide virtual education and support to patients, families, and caregivers regarding symptom management and end-of-life care
- Liaise with field nurses, physicians, social workers, and other team members to ensure comprehensive care delivery
- Document all patient interactions accurately in the electronic health record (EHR)
What is Required (Qualifications):
- Active RN license in a Compact State (eNLC)
- BSN required from an approved Nursing education program
- Minimum of 2 years of experience in hospice, palliative care, or triage
- Proficiency in Epic or Home Care EMR
- Strong communication and interpersonal skills to build rapport with patients and families virtually
How to Stand Out (Preferred Qualifications):
- Experience working in a telehealth environment
- Familiarity with virtual care technologies
- Additional certifications in hospice or palliative care
