04/11/2025 - Provide confidential service to residents in long-term care facilities while conducting appropriate fact-finding investigations in order to understand an issue from all perspectives better, while maintaining allegiance to the client.
- An ability to work cooperatively with individuals who live in long-term care facilities, the Regional Ombudsman, and appropriate providers.
- An ability to build cooperative and trusting relationships with residents of facilities as well as individuals.
- Complete travel to various sites (e.g. homes, long-term care facilities, relative meetings, etc) as long as the employee and supervisor feel the employee can do so safely.
- Inform residents, family members, staff, and other interested parties about residents’ rights, community services available to them, laws, regulations, and standards that govern long-term care facilities, and good health and safety practices.
- Successful completion of the Long-Term Care Ombudsman Program Training Program and State Long Term Care Ombudsman's Certification as a Long-Term Care Ombudsman (continuous education/training is also required for this position)
- Respond to or direct requests for assessment, evaluations, and referrals for assistance. Provide consultation to community senior organizations and community members regarding long-term care and lower levels of care as a means to divert nursing home placement.
- Immediately report all actual or suspected incidents of abuse and neglect to the Regional Ombudsman or other entity as required by law and policies if the city and state.
- Maintain current written records of contact with residents and visits to facilities by maintaining records in accordance with guidelines of the Office of the State Ombudsman.
- Establish a relationship with Resident members and attend Resident Council meetings when invited. Assist residents and their families and friends to be actively involved in their care and treatment through resident and family councils.
- Establish a relationship with Family Council members and attend Resident Council meetings when invited.
- Performs other related duties as assigned.
- Demonstrated experience with state Ombudsman programs, including certification and training requirements, and adherence to the State Long-Term Care Ombudsman Program.
- Demonstrated experience providing social services in a non-profit or public health setting.
- Experience working with people of diverse backgrounds and cultures.
- Excellent communication skills, including verbal, written, and listening abilities, with the capacity to address diverse audiences effectively.
- Strong ability to build and maintain effective working relationships with employees, City officials, community agencies, community members and the general public.
- Exceptional problem-solving skills, including the ability to identify core issues, develop options, and determine effective outcomes even in complex situations.
- Proficiency in office computer software, including word processing, spreadsheet, and presentation tools.
- Demonstrated ability to exercise independent judgment in planning work, formulating recommendations, and making decisions.
- Capacity to maintain strict confidentiality and demonstrated ability to act independently while also being a team player.
- Ability to work cooperatively and in collaboration with the City of Evanston Regional Ombudsman toward local and state initiatives.
- Be presently employed by a long-term care facility;
- Have any current fiduciary interest, either direct, indirect or implied, in any long-term care facility;
- Be assigned to work in a facility in which a member of the immediate family resides;
- Conduct or engage in political or religious activities in the assigned facility;
- Use the position for any financial benefit, direct, indirect, or implied;
- Provide any hands-on care to any resident; or
- Bring in or distribute in any facility any form of alcohol, tobacco, or medications including such items as over-the-counter medications. SALARY RANGE: $ 38.462-$49.442/hourly

*Starting salary is dependent upon qualifications, but in most cases is no higher than the midpoint for the range.

NATURE OF WORK :
The Part-Time Ombudsman plays a crucial role in promoting the well-being and protecting the rights of seniors living in long-term care facilities and home and community settings. The position serves as an advocate and point person within the City, leading issue and service advocacy efforts related to seniors and individuals with disabilities. The Part-Time Ombudsman assists with the Long-Term Care Ombudsman program, while also conducting community education, and complaint investigations. The role involves collaboration with other City departments and outside agencies, requiring attendance at meetings during non-traditional hours.
ESSENTIAL FUNCTIONS (Specific assignment will include some or all of the following):
MINIMUM REQUIREMENTS OF WORK :
Must possess a bachelor's degree from an accredited college or university in public health, aging, social services, or a related field
Must possess a valid state driver's license and safe driving record.
Must possess one (1) year or more years of work experience in a long term care setting, non-profit setting, a public health setting or an Ombudsman program OR a combination of training and experience that provides the following knowledge, skills, and abilities.

PHYSICAL REQUIREMENTS OF WORK :
The Part-time Ombudsman can expect to work in a primarily sedentary position but may occasionally need to exert force when lifting or moving office-related objects. The employee will work indoors, protected from outside weather conditions, but occasional outside visits may be necessary for program management or emergency responses.
The position may involve responding to urgent situations within the City organization or with clients, some of which may occur outside traditional work hours. The role may require handling emotional, non-rational, or psychological situations.
SUPERVISION :
Under the general guidance of the City’s Regional Ombudsman with management from the Health and Human Services Director, the Part-Time Ombudsman operates within the overall City's mission, framework, and policies. The position is responsible for prioritizing tasks and leveraging appropriate resources to achieve organizational goals. Compliance with City codes, state and federal regulations, and departmental policies is essential. Performance evaluations are conducted regularly based on technical expertise, goal achievement, problem-solving abilities, communication skills, customer service, and adherence to this job's classification standard.
PUBLIC CONTACT :
The Part-Time Ombudsman regularly engages with City officials, service providers, employees at all levels, community groups, the public, and federal, state, and local agency representatives.
