      Job Summary

      The Patient Care Coordinator serves as a liaison between CHOC Health Alliance members/families, medical providers, and behavioral health providers. Identifies and reduces barriers to members' care, assists the members in locating and accessing resources, and educates the members on the importance of accessing care promptly. Serves as the primary point of contact for the member and/or family and will be an integral part of the member's interdisciplinary care team. Provides support to the members as they navigate the health care system and works closely with member's case management team to resolve access, medical, and psychosocial related issues.

      CHOC Health Alliance is a Provider Network for CHOC’s CCS and Medi-Cal/CalOptima patients. Ideal candidates will have experience working in the medical field and/or in a community-based setting, with underserved populations.

      Pay Range

      Minimum $23.77 Midpoint $31.49 Maximum $39.21

      Rate of pay is determined by various factors, including but not limited to knowledge, skills, competencies, experience, education, as well as position requirements.

      Experience

      Required: Minimum one (1) year of related experience.

      Preferred: Two (2) years of related experience; Experience in a medical setting and/or working in the community with underserved or underinsured populations.

      Education

      Required: High School Diploma/ GED or equivalent.

      Preferred: Associate degree in healthcare related field.

      Training

      Required: None required.

      Specialized Skills

      Required: Ability to maintain accurate records and documentation. Ability to effectively interview members to determine the client's strengths, problems, functional status, goals, and need for specific services/resources. Ability to develop rapport, establish and maintain effective interpersonal relationships with internal and external contacts at all levels. Ability to work with case management staff to expedite the resolution of member concerns. Communicate effectively, both verbally and in writing with individuals from varying cultural and ethnic backgrounds.. Ability to speak, read, write, and understand English effectively at a level appropriate for the safe and effective performance of the job.

      Preferred: Prefers knowledge of HMO, Medi-Cal/Medicare and health services. Knowledge of principles and practices of health care service delivery and managed care, Medi-Cal eligibility and benefits. Knowledge of Principles and techniques to serve the SPD population in diverse social and ethnic groups. Ability to utilize effective charting practices and guidelines. Demonstrated ability of proficiency regarding; personal computers, keyboarding, and appropriate software to produce correspondence, charts, spreadsheets, and/or other information applicable to the position.

      Preferred: Bilingual in English/Spanish, English/Vietnamese, or English/Farsi
