      Patient Educator

      Do you find fulfillment in connecting patients with information that will help them make an informed choice about their treatment plan? A Patient Educator role may be right for you!

      As a Patient Educator, you will be responsible transforming dialysis care by specializing in new patient education, enabling patients to make an active and informed modality choice – by teaching and educating patients and teammates on ESRD modality options including lifestyle benefits, challenges, techniques and alternatives. The educator is responsible for understanding the patient’s values and adjusting the education to meet the needs of the patient, leveraging educational materials.

      What to expect in this role:


      Educate and Inform Patients

      Educate and inform patients, patient families, and teammates about treatment options and overall dialysis education. Utilizing standard materials to communicate to patients, patient families, and teammates. Provide assistance with dialysis transitions as required. Work with on-site inter-disciplinary team to identify and educate potential and existing patients on specific modality options. Coordinate and communicate with the clinic interdisciplinary team including but not limited to the in-centers nurses, social worker, dietitian and Home nurses to plan education delivery and transition steps, when appropriate, and to complete the education process.

      Trend Analysis

      Monitor, track trends, and report key metrics and key drivers in decision.

      Travel

      Geography dependent, but up to 75% in the local region


      The shining star in this role should have:

      Required Qualifications

      • Demonstrated ability to adapt messaging and education approach to the individual patient’s circumstances and learning styles.

      • Experienced in teaching the adult learner with demonstrated ability to recognize and customize education to individual learning styles.

      • Experience in project management to coordinate the patient experience with teammates while systemically tracking patient progress through the education process.

      • Ability to cultivate connections with patients, clinic teammates and physicians.

      • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required; functional proficiency with DaVita applications including but not limited to SNAPPY, Reggie, Laboratory, required within 90 days of employment or position change.

      • High School Diploma, Vocational/Technical school certificate degree, Associate’s degree or Apprenticeship.

      Preferred Qualifications (a.k.a. nice-to-haves)

      • Bilingual

      • Minimum of one (1) year’s nephrology experience, home dialysis experience.

      • Demonstrated knowledge of chronic, peritoneal, nocturnal, and home hemodialysis.

      • Experienced in case management, home health, disease management, or hospice.

      • Minimum of one (1) year experience educating patients on ESRD and CKD programs.

      • Clinical license or certification; dialysis specific if possible

      Salary/ Wage Range

      $21.00 - $30.00 / hour

      Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
