What to Expect (Job Responsibilities):
- Conduct patient outreach through telephonic and in-person home visits to encourage engagement in care
- Spend approximately half of the time in the community connecting with patients in various settings
- Document patient interactions in electronic medical records while adhering to HIPAA standards
- Communicate complex topics among all parties involved in patient care, including family and caregivers
- Collaborate with Community Health Workers, Social Workers, and clinicians to support complex patient needs

What is Required (Qualifications):
- Track record of success in healthcare-related engagement, sales, and/or marketing
- Strong verbal and written communication skills
- Proficiency in technology, including Excel and EHR/EMR platforms
- Understanding of Medicare and Medicare Advantage programs
- Access to reliable transportation for travel within assigned territories

- Fluency in Spanish (written and oral)
- Demonstrated ability to operate independently in a fast-paced environment
- Experience in building relationships within the healthcare community
