What to Expect (Job Responsibilities):
- Reach out to patients after recent hospital visits and connect them with a registered nurse
- Obtain and document basic information regarding patients' recent hospital stays
- Review and prioritize patient census for effective follow-up
- Explain services and enroll patients into appropriate programs based on their needs
- Address inquiries and assist in scheduling initial and annual appointments
What is Required (Qualifications):
- High School Diploma or equivalent
- Minimum of 1 year of experience in a call center, sales, healthcare, or customer-focused environment
- Strong communication skills to interact effectively with patients and team members
- Ability to manage multiple tasks and prioritize workload
- Proficient in data entry and documentation in designated systems
How to Stand Out (Preferred Qualifications):
- Experience working in a healthcare setting, specifically in patient enrollment or care transitions
- Familiarity with healthcare programs and services
- Demonstrated ability to work independently and as part of a team
- Previous experience in a customer service role with a focus on patient care
- Knowledge of medical terminology
